Smith faculty, students and staff can use this form to:

Please check the Five College Library Catalog for availability and borrow Smith videos in person from the Neilson or Josten Library circulation desks.

You may also borrow videos in person at any Five College Library.

Guidelines
Deadline:	5 working days in advance
Limit:	3 requests per week
Loan Period:	one week with no renewals
Pick up and return:	We will send an email when the item arrives - it will be held for 3 days at the Neilson Circulation Desk
